Our website and contact form are for business inquiries. Do not include patient names, diagnoses, insurance details or other protected health information (PHI) in the form. If we provide billing services to your practice, PHI is handled under a separate written agreement with you, such as a business associate agreement, and not through this website.
We do not sell the personal information collected through this website. We share it only with service providers that help us run the site and receive inquiries, such as our website host and our form-processing provider, and when required by law. Form submissions are sent to us through a third-party form service.
We keep inquiry information for as long as needed to respond to you and manage our business relationship, then delete or anonymize it. We use reasonable administrative and technical safeguards, including encrypted (HTTPS) connections, but no method of transmission over the internet is completely secure.
