Primary Care Billing Services

High volume, many payers and constant coding judgment calls. We keep primary care claims clean and A/R moving.

Bill For You RCM manages billing for primary care and family medicine practices: E/M code review, preventive and wellness visit billing, chronic care and care-management codes, eligibility checks, claim submission, denial appeals and A/R follow-up. Fee: the greater of $650 a month or 2.99% of collections.

Why primary care billing is hard

  • Volume and variety: acute visits, chronic care, preventive care and procedures in the same day.
  • E/M leveling: codes depend on medical decision making or total time, and notes must support the level.
  • Modifier 25 and bundling: same-day procedures and preventive services create frequent denials.
  • Many payer rules: Medicare, Medicare Advantage, Medicaid and commercial plans handle preventive and wellness benefits differently.

  • Modifier 25 missing or not supported by documentation.
  • Preventive and problem-oriented services coded incorrectly together.

Frequently asked questions

What is modifier 25 and why does it cause denials?

Modifier 25 marks a significant, separately identifiable E/M service on the same day as a procedure or preventive visit. Payers often deny it when documentation does not show a distinct problem-oriented service, so notes and diagnosis links must support it.

How is the E/M level chosen?

Office E/M levels are selected by medical decision making or total time on the date of the encounter. The documentation must support the level billed.

How much does primary care billing cost with Bill For You RCM?

The greater of $650 a month or 2.99% of monthly collections, with $0 setup and no long-term contract. See the pricing page.
